New Drug Rehab Model Used in Second Chance Program in Albuquerque

A new drug rehabilitation program that uses long-term nutritional and sauna-based detoxification - with no alternative drugs - is achieving high success statistics in a prison setting. Run by Rick Pendery, the national executive director of the Second Chance Centers, and Joy Westrum, the president of the Second Chance Program, the program has been running for several months in a secure setting in Albuquerque, NM.

Westrum explains that the purpose of the program is to "provide secure, long-term rehabilitation for substance abusers in the criminal justice system, and then to transition them back into society so they can lead a stable life free of crime and drugs. The program includes the administration of vitamins and mineral supplements twice a day, drills designed to extrovert the inmate's attention, and physical techniques designed to decrease the physical trauma of withdrawal that are administered by other inmates. The entire process is supervised by a staff member trained in the process."

This long-term detoxification process has been used throughout the world, not only to assist people in getting off drugs, but also to remove chemicals and other toxins from the system. For example, it was used by the New York Fire Department and others who worked at Ground Zero after the disaster of 9/11.

"When the person has completed the long-term detox, the metabolic residuals from years of substance abuse are eliminated from the body. We have found that the detox measurably decreases the craving to take more drugs," Pendery explains. "The also appear more healthy, as witnessed by physical indicators, such as improved skin tone and clear eyes, as well as social indicators like higher activity levels, better communication skills, and so forth."

Those on the program also engage in what are called Therapeutic Training Routines, which are known to increase the inmate's attention and awareness of his present environment, raise his ability to comfortably confront others and examine his own life, and raise his ability to give and receive communication. "These routines consist of actual communication drills that were developed by L. Ron Hubbard, the founder of a newly recognized religion called Scientology," explains Pendery. "Second Chance is a secular program that uses protocol-based research from Mr. Hubbard, but Second Chance is non-religious by nature and is not part of the Church of Scientology. Developing better communication skills, eliminating toxins stored in the body, and learning tools that more effectively help someone learn and apply what they learn are examples of some of these protocols. Our center provides Sunday Catholic and Protestant services, and practices an official policy of respecting the religious beliefs of others. The inmates here are encouraged to pray, read the Bible, meditate, or take part in any religious activity."

Concludes Pendery, "The results of this part of our program really pull the individual out of his 'drug life.' The remaining steps help stabilize him so he can successful reintegrate into society."

He outlined how the various parts of the Second Chance Program fall within what is considered to be the best practice in substance abuse treatment, saying, "The Second Chance program uses a holistic approach of exercise combined with the use of nutrients to replace metabolic deficiencies. Flipping Houses Ethics: Ethical Rehabbing

In wrapping up my article series on the ethics of flipping houses and real estate investing in general, I?d like to share a word on rehabbing houses ethically.

First, for those who may be unfamiliar with the term, ?rehabbing? a house is simply the act of making repairs and/or renovations to put the house into ?fair market condition? ? or, as I prefer it, ?like new? or ?better than new? condition.

The purpose is typically to build value into the property where value currently lacks ? either for the sake of equity or profit at resale.

When I rehab a house, I tend to go over and above, spend more and do more than your average real estate jockey.

When I rehab homes, I like to do them ?right.? I don?t cut corners or hide things that should really be fixed, and if you strive to be an ethical investor, neither should you.

If I encounter something that could be costly, I always explore all of my options as long as the final result will provide my customer, the ultimate homeowner, with a quality product. Though I?m not opposed to finding the cheapest way to get things done, I want the job done properly rather than just covering up defects which should really be fixed. And whenever I'm helping newer investors learn how to get started right in the house flipping business, that's exactly what I teach them.

Of course, just like settling all of your deals, rehabbing houses properly can be difficult at times ? especially if you find yourself working on a shoestring budget.

But in the end you have to sleep well at night. And you can sleep well knowing that you?ve virtually eliminated any risk of a lawsuit from an angry homeowner and created a good name for yourself in the community to boot by rehabbing something the right way. As with settling your deals, this type of reputation can go a long way, particularly if you are focused on rehabbing homes in a particular area. And shoddy work yields a shoddy reputation.

Lastly, I must caution those of you who know the types of rehabs that I do today not to get caught up in thinking that you must do full-blown gut rehabs to do them ?right.? This is just my preference, and I?ve told you in other articles why I personally prefer the ?gut rehab?.

You can do cosmetic ?paint and carpet? rehabs and still do them ethically as I?ve described. I still like to complete one of these myself every once in awhile, as it?s nice to get an easy deal here and there.

My main point is this. If you know there?s a leaky pipe in a wall that no one else can see, fix it! Don?t hide or gloss over necessary repairs, even if you think no one will find out for a long time. If you make this a business practice, it will catch up with you in one way or another.

How to Strengthen Your Rotator Cuff for Injury Prevention and Rehabilitation

Chances are if you are at all familiar with your rotator cuff it is because you have injured it at some stage. Unfortunately the group of muscles that make up the rotator cuff are quite vulnerable to injury, especially when subject to sudden stresses and strains that occur in some sports. For example, throwing a ball can be very stressful to the rotator cuff. That being said, it?s not only those participating in sports activities who are at risk of rotator cuff injury ? the rigors of just regular daily activity can also exact their toll on the rotator cuff, especially where excessive repetition is involved.

So how do we avoid injuring our rotator cuff and if it?s been injured in the past then how do we ensure the quickest recovery and minimize the likelihood of injuring it again?

The answer lies in strength. In truth there, are a number of factors but strength is a major factor and the one we will consider in this article. Quite simply: a strong rotator cuff is more likely to withstand the demands of everyday life as well as the exertion of sporting activities. Strong and healthy muscles are less likely to be strained or torn and are more resilient to higher than usual demands being placed on them. Injury recovery will also be quicker.

So how do you strengthen your rotator cuff? The same way you strengthen any muscles: exercise. There are a number of exercises that directly work the muscles of the rotator cuff and one of the easiest and most effective to do is the one I will describe below.

Before we get to the exercise I should insert a quick few words of warning: if you are already suffering an injury you should check with your doctor before performing any exercise that works the injured area. Also, if you feel any pain while exercising then back off a little ? either reduce the weight or resistance you?re using or decrease the range of motion to within pain free limits, or, if neither of these actions eliminate the pain then you should not perform that exercise until you can do it without pain.

With that out of the way, here?s how to perform the exercise:

To exercise the rotator cuff on your right hand side you lie on your left side, on a bench if you have one or otherwise the floor is fine. Position yourself so your upper right arm lies along the length of your upper body and is bent 90 degrees at the elbow to make an ?L? shape with your forearm resting across your body. Place a folded up a hand towel and position it under your right elbow so your upper arm is supported and is parallel to the floor. This is the starting position.

Take a very light dumbbell (it is essential to start with a very light weight especially if recovering from an injury) and as you exhale slowly and deliberately rotate your forearm to a vertical position all the while keeping your elbow resting on the towel and your upper arm along your body. So the finish position has your upper arm along your body and your elbow still bent at 90 degrees with your forearm vertical and your fist pointing at the roof. Lower slowly and repeat for at least 12-15 repetitions for each set.

This is just one simple exercise, easily and comfortably done by anyone, that will increase the strength of your rotator cuff. With a strong rotator cuff you will be less likely to injure it and more likely to recover quickly if you do happen to sustain an injury to the cuff. You can also use this exercise as part of a post-injury rehabilitation program but be sure to do so only under the advice of your doctor or physical therapist.

Veterans Stop Procrastinating: File Your Claim For Vocational Rehabilitation With The VA

OK so you're out of the service, back home and everything is right with the world. Or is it? After a month or so, you're sitting around at home and you realize - you really need to get back to work and make some money- duh. Your significant others are kind enough to remind you of this and your disability severance pay is already running through your budget like water. Now what? Where are all the jobs that were supposed to be there? Where do you start?

One place to begin is the local employment service office. But the jobs there are often low paying and demeaning with a high turnover. Who wants to be an unskilled laborer 40+ hours a week, with no benefits - and work for someone who's never been anywhere or done anything and treats you like a moron? And, forget about medical services for your children or a visit to the dentist. After the work week is done you spend the weekend in pain, nursing your painful back and tend to drink too much as a result.

Alright , so maybe the answer is to find another career - like you were trying to build in the military, until you hurt your back and they forced you out.

A career is much better than a job, since you will obtain job skills that are in demand in the civilian job market, you will generally earn much better income and will very often receive some RESPECT along the way! Another important difference is that you will be using some of your unique strengths and talents again and going to work Monday morning will not be such a dismal experience.

You begin to realize that the ideas and plans you made in the service are not realistic and the job market is nowhere as good as you had hoped. You also begin to understand the handicapping affects of your service-connected disability. Many civilian employers are not enthusiastic about hiring you for any type of physically demanding job, as soon as they see that you were separated from the military, because of fitness for duty standards.

This may be an excellent time to apply for vocational rehabilitation benefits through the VA. You will generally need to have a service-connected disability rating of 20%, or higher from the VA, to qualify for an assessment and determination of eligibility. You can also apply while on active duty, awaiting a separation for disability, or shortly after discharge, by providing medical documentation from the military and requesting a "Memorandum Rating". In this case the VA Rating Specialist does a quick review of your Service Medical Record and makes an unofficial determination that you probably qualify for a 20% or higher rating. This decision in turn, allows the Vocational Rehabilitation Division to work with you, as if you already had your permanent rating. You need to file an application online, or by filling out and submitting VA Form 28-1900. Call toll free 1-800-827-1000, or download online from www.va.gov/vaforms/.

The VA Vocational Rehabilitation Counselor will make a determination that your service-connected disability creates an "employment handicap" for you (or not -you can always appeal, if necessary). This in turn basically means you are prevented from "obtaining or maintaining" suitable employment because of your service-connected disability. What is suitable employment for you? Good question! Basically, it is substantial (skilled) employment which is consistent with your "demonstrated" interests, aptitudes and abilities and for which there is a job market. The VA will evaluate interests, aptitudes and abilities through your military training and employment; through your civilian education and employment history; and through standardized testing.

It may be that you have done a great deal of soul searching and know exactly what you'd like to go into, or get training for. Or it may be that you haven't got a clue - either way, you can get assistance towards defining a new career goal, as well as possible funding for further education, or career training. The VA is also able to provide direct employment assistance if it is felt you already have marketable job skills, but you have a need for some direct assistance ( ie. resume, etc).

Finally, you need to be realistic with yourself, as well as the VA and employers! Don't ask for training to be a carpenter, for example, if you have a back disability. Whatever career goal you establish, has to be reasonable in terms of your health challenges, your capabilities and the civilian job market. Remember, you really only want to be going through vocational rehabilitation once - then it's on to bigger and better things!

Flipping A Rehab House

What is "flipping" a rehab? It is making an offer on a fixer-upper, and then selling the contract. It can mean fast profits with little risk, and very little money invested. The downside? You may make less profits than if you fix it up yourself, and can spend a lot of time looking for the right properties.

We are talking about classic real estate "flipping" here. You never really own the property or have any of the headaches that go with fixing up a place. Instead, you find properties with a lot of profit potential, get your offer accepted, and then sell your "place" for a profit. What you need to make money this way:

1. A good eye for value.

Without any idea of what homes in an area should sell for, you will waste a lot of time. You should be able to look at a home and say, "It should sell for about x amount once it is fixed up." Of course, you'll do research to arrive at a more accurate estimate once you target a particular property.

2. An idea about repair and improvement costs.

Again, you can waste a lot of time if you don't know what it costs for certain common repairs and improvements. Guessing that a roof repair will be $2,000 and then getting a quote for $10,000 means a blown deal after perhaps days of effort.

3. A few good contacts with investors who like fixer-uppers.

Once you get that offer accepted, you need to have some investors to call right away. You have signed a contract with deadlines. This is not the time to start introducing yourself to other investors.

4. The line "or assigns" in every offer.

You put "or assigns," or alternately "or my assigns" after your name in every offer (talk to an attorney or experienced investor to see what language is common in the area). This gives you the right to assign the contract to another buyer. If the seller asks about this, you can tell him that you may want to bring in a partner or turn the project over to your partner, but that all the same terms apply, so there is nothing to worry about.

5. Financing contingencies and other escape clauses.

If you can't find a buyer to assign the contract to, you will have to complete the transaction yourself. Unless, of course, you can't, and you gave yourself a way out for just such a possibility. These escape clauses might include making the deal contingent on getting a certain interest rate, or on getting the approval of your partner within a week.

Flipping a Rehab - An Example

You have been in touch with a few investors that invest in fixer-uppers, or "rehabs". You know what kind of properties they like, and how much profit they expect to get out of a project. Now you start driving around town, looking for fixer uppers.

You find three for sale through realtors, two for sale by owner, and several homes that appear abandoned and might have owners looking to sell. You take a lot of notes, and you go to the courthouse to find the owners of the empty-looking houses. Once home, you start making phone calls and talking to owners and real estate agents - and taking more notes.

Starting with the one that has the most potential, you look at the properties. For each, you lay out a plan on paper, then estimate the costs. Using the formula laid out in Number 7, you estimate the eventual sales price, and subtract from this the costs and the profit you need to make you and the final investor happy. This gives you the highest price you can offer.

One of your offers is accepted. It is a house that will be worth $156,000 when it is ready. The costs will run about $16,000, and the buyer accepted your offer for $114,000, leaving a projected profit of $26,000. Your good faith deposit is $500.

Now you get busy talking to your investors. One agrees with your assessment of the potential, thanks to your detailed plan showing the repairs and improvements, as well as your market analysis showing what similar homes have sold for recently. He likes to make $20,000 on any given deal, so he offers you $6,000 to have the contract assigned to him.

Of course it can get more complicated than this, and you are not likely to get an investor who will pay you prior to closing, so you'll have to wait a few weeks. However, notice that you only invested $500 and your time to make that $6,000. Also you may not want to handle a dozen fixer-uppers if you were doing them by yourself, but flipping a rehab house is a lot less work, so you could be working on several such deals more easily. You can see why some investors concentrate on flipping properties.

Don't let the Better Business Bureau choose your contractor.

You may be tempted to call your local Better Business Bureau to see if they have anything on file for a prospective contractor. This might give you reason to eliminate a contractor from your list, but it won't tell you what you need to choose one. Remember, the Better Business Bureau is primarily a collector of complaints. The lack of complaints against a contractor doesn't mean the contractor is qualified.

Don't over-value experience.

It's easy to compare contractors by the years of experience they have. However, experience alone doesn't always translate directly into high-quality work. A contractor could have been doing substandard work for his entire career. Go beyond the numbers and evaluate their work.

Beware of high-pressure pricing tactics.

Both of you are professionals and there's really no need for the use of force. If you feel pressured when negotiating with a potential rehabbing contractor, don't be shy about asking him to back off. If that doesn't work, don't hesitate to look for another prospect.

Beware of big down payments.

You should be able to negotiate a reasonable down payment and a fair payment schedule. Typically, you should expect to pay no more a third of the total rehabbing cost in a down payment, with the remainder paid out as work is completed. If the contractor asks for most of the money up front, that could indicate he has a shaky financial condition.

Good communication is a must.

You and the contractor should easily find yourselves on the same page. You're going to be spending a lot of time working with each other. Being able to communicate with each other accurately and effectively will make the rehabbing project go much more smoothly.

The lowest bidder may not be your best bet.

Price should not be your only consideration for choosing a rehabbing contractor. You should decide ahead of time exactly what level of services you want done. Your goal should be value. If the contractor you feel best about has a bid that's too high, you can always ask for a lower price.

A good general contractor will make your rehabbing project go more smoothly. A smoother, more efficient rehab means more profits for you. These tips should help you find a contractor that not only is competent, but is also enjoyable to work with.

Finding Contractors to Rehab Your Properties

Contractors are your right-hand men in this business. Without them, you?ll never be able to grow your rehabbing business. They are the ones that will be rehabbing your properties and ultimately, making much of what you do as a rehabber possible. Dealing with contractors is my favorite part of the rehabbing business, and that?s primarily because they can be such a great source of entertainment. (Because of them, I now have plenty of funny stories to tell.) Working with contractors requires a fair warning about what you?ll encounter with them, so prepare yourself. Never be surprised by anything that a contractor does. Never assume that they?ll do exactly as you hope. Keep this in mind whether they have worked for you one time or thirty times. And most importantly, remember that you are the one in control ? always!

I have not always turned out a finished property that I was proud of. In the beginning, I hired backyard handymen, rather than true contractors, because they were less expensive. I was not savvy enough to know that their work was crappy. They were small-time players who half-fixed everything and relied on super glue and duct tape to get them through their day. They did meet nor even know housing codes. They cut corners whenever they could. I believed them when they told me they had the expertise to fix anything that needed done: plumbing, electrical, furnace, etc. Boy, was I gullible!

These contractors would lie, cheat, and steal in order to make me believe that they were competent. In the beginning I was not experienced enough to know what quality work looked like, I did not realize that I was not selling a quality product. I soon had an epiphany: I was paying these small-time contractors $5 less and getting $1 results! I quit using backyard handymen. I began researching to find people who knew what they were doing and I was willing to pay to have these quality people. It was one of the most important lessons that I have learned. Professionals do a quality job within a reasonable time frame. They are priceless. I cannot emphasize enough the importance of a good, professional contractor.

You really need to be certain that home repair work is being done right. I am not suggesting that you learn how to do the work yourself (which may not be the best use of your time) but do become aware of what a job well done looks like. I use inspectors to survey a property before the work is done and after. I also have very specific contractor guidelines to insure the quality of my properties. If the contractor hasn?t followed the guidelines, they haven?t finished their job! You want to be able to turn over the best product available to your customers and you need to hold your contractors responsible for their work.

Joe Crede Opts For Rehab Instead Of Surgery

<p><strong>Joe Crede, the White Sox third baseman, has opted for a six to eight week rehab program instead of surgery for the two herniated discs in his back. Till a few days ago, there was a lot of speculation that Crede might go for microsurgery. Unless great effort has been put into awareness training, developing knowledge about the experiences and circumstances of minority groups, and the establishment of programs to deal with the special needs of different groups of oppressed people, services will simply perpetuate institutional discrimination.

Gay Alcohol Treatment Programs Not Adequate

U.S. researchers Lohrenz et al (1978) found that 37% of homosexuals experienced discrimination from alcohol treatment program staff while Fifield, De Crescenzo & Latham (1975) discovered that 75% of homosexuals who are recovering from alcoholism believe that mainstream drug rehab and alcohol rehab program are not geared to treating homosexuals and do not provide an accepting and supportive environment. Because of discrimination homosexuals are less likely to attend an alcohol treatment program and drug addiction treatment program unless, that is, they are 'passing.' In this case, if the clinician does not bring up the subject, one of the major causes of their problems will be ignored. Rofes (1989) says: By ignoring the special problems that a lesbian alcoholic, for example, presents, an alcohol treatment program will be doing a service to no one. Their alcoholism treatment of the individual will be less than adequate and may tend to intensify the woman's feelings of isolation and 'difference.' Only by bringing the issue into the open and addressing the woman's lesbianism as an aspect of her life which she needs to feel positively about, will the program be truly effective.

Avoiding Coming Out

Shernoff & Finnegan (1991) discuss the case of a lesbian who is hiding her sexuality, then stress: It is the responsibility of each alcoholism treatment counselor to take the lead in this area the same way alcohol treatment counselors routinely question early family history, dynamics of shame, denial and spirituality. By omitting questions about sexual orientation, or the more subtle questions about sexual or affectional feelings or fantasies for a person of the same sex, the counselor is not obtaining information about all the possible contributing factors for achieving and maintaining sobriety. While Hellman et al (1989) note: Therapists may fear causing anxiety by asking patients about sexual orientation because of discomfort with the subject. Problems Faced In Gay Alcohol Treatment Programs

American surveys, referred to by Hellman et al (1989), reveal a list of complaints about mainstream provision ranging from

? heterosexual bias in alcoholism treatment and evaluation (including either focusing primarily on sexual orientation when inappropriate or ignoring important factors linked with sexuality)

? ignorance about lesbian/gay issues and discomfort at approaching matters of sexuality

? ignorance about the inter-relation of homosexuality and alcohol abuse

. Neisen & Sandall (1990) worked at a program designed to offer alcohol treatment to chemically dependent lesbians and gays. They list their clients' experiences of non-gay drug rehab or non gay alcohol rehab, which include:

difficulty in being open about their sexual orientation due to fear of staff/client harassment,? staff telling them it wasn't acceptable to discuss sexual orientation

some were forced to disclose their sexual orientation

as soon as their sexuality was known, some were discharged

some said that after disclosure the alcohol treatment they received was different due to an atmosphere of condemnation

some feared that if their sexual orientation was known about this would receive more emphasis than their chemical dependency

some addiction treatment programs were not happy having their partner attend a family program.

Citing Morales & Graves (1983) and Hellman, Stanton, Lee, Tytun and Vachon (1989), O'Hanlan (1996) notes:

? the majority of detox and drug rehabilitation and alcohol rehab programs were insensitive to issues of sexual orientation and did not, generally, encourage its disclosure

? homophobia limits the success of recovery and alcohol treatment for lesbian substance abusers (Hall, 1990; de Monteflores, 1986)

? failure to acknowledge sexual orientation makes relapse more likely (Cabaj, 1992)

? lesbians were more likely to attend alcohol treatment program services which address lesbian social issues and provide lesbian counsellors (Hall,1986, 1990, 1992, 1993, 1994; Morales & Graves, 1983).

Inclusion of families in addiction treatment and alcohol rehab program is now an acceptable way of supporting those coming off alcohol dependency (Nardi, 1982; Shernoff & Finnegan, 1991). You make your profit when you buy. Pure and simple! I will always tell my coaching clients at my workshops that this is the most valuable advice that they will ever receive from me. The added value from the renovation is the icing on the cake.

Always research your market well and ensure that the potential selling price is achievable when all costs and profit margins are taken into consideration. Never, ever let yourself be ruled by your emotions when buying. You must always allow for buying, selling and closing costs. Where possible, your purchase price must be sufficiently below market value to at least allow for these costs. Even better, the price should be low enough to allow for closing costs plus rehabbing costs.

The following formula will allow you to assess the real purchase cost of a particular home that you may have in mind.

Starting with the final selling price, work backwards and deduct selling costs, profit margin, renovation costs and buying costs. This final figure is how much you should be prepared to pay.

BIG MISTAKE #7: FAILURE TO UNDERSTAND YOUR TARGET MARKET

How often have you walked into a home and been totally horrified by the d?cor? Yet this is another common mistake made by renovators. They let their emotions get in the way and decide what is good for them is good for everyone else.

Nothing goes exactly to plan when renovating, so don't panic if you exceed your budget by small amounts. Allow for a buffer to cover any surprises (usually 10 to 15 per cent). I certainly fell into the 'do too much' trap during my early projects.

Always have your costs estimated accurately prior to purchasing a property. Later on I will talk about contract of sale conditions. I encourage you to renegotiate the price should the inspection uncover any major defects - or better still, walk away.

My budget parameters are 10 per cent of the purchase price for houses. This can be stretched to 15 per cent if it means that a great profit can be made. These are personal preferences that have worked for me. They are in no way set in concrete and readers should exercise their own judgment. There is no right or wrong way in this business.

The percentages that I allow are in keeping with my primary objective of obtaining the biggest bang for your buck in the shortest possible time and with the minimum amount of money possible.

Conclusion: Understand that things can and do go wrong in this business. Now that you are aware of the pitfalls, you will be better prepared from the outset. I almost started laughing at the question as to what a gay friendly alcohol rehab was. What did an alcohol rehab have to do to be gay friendly. Did the alcohol rehab have to have special interior decorating, bright color and style? We know better, but ti was not always like that.

Historically, most alcohol rehab programs were designed to treat the disease of addiction and alcoholism, regardless of sexual orientation. As time passed and addiction treatment professionals gained additional experience in addiction treatment, specialty programs began to spring up all over the country. There was an alcohol rehab program for men, an alcohol rehab for women, an alcohol rehab program for Christain's, but it appeared that most of the alcohol rehab programs expected the gay population to "just fit in".

While this alcohol treatment program approach worked for some, many gay, lesbian and bisexual's felt out of place, scared and discriminated against, making effective addiction treatment almost impossible. In order to treat the gay population effectively, one of two things had to happen; either design an all gay alcohol rehab or an alcohol rehab termed a gay friendly alcohol rehab.

While an all gay alcohol rehab is self explanatory, what did an alcohol rehab program need to do to be gay friendly and provide effective alcoholism treatment. The answer had a complex solution as the typical alcohol rehab approach needed to change a bit. First, the entire addiction treatment staff needed to be educated and sensitized to the needs of the gay individual. They needed to be taught about their lifestyle, homophobia, heterosexism, coming out and the host of issues the gay community faces. Each staff needed to be assessed as to their homophobic attitudes and educated accordingly.

Secondly, a group specific to gay addiction treatment issues needed to be developed, with a therapist trained in the areas specific to the gay population. This alcoholism treatment group would meet several times a day in conjunction with all the other addiction treatment services offered in the alcohol rehab.

Lastly, for a gay friendly alcohol rehab to be effective in the treatment of alcoholism or drug addiction, it must have a comprehensive family recovery component. THis family recovery component needs to be much more than a family session once a week. There needs to be at least a four day period of time that the entire family is together at the alcohol rehab with the family therapist to resolve any outstanding issues within the family system. Issues within a gay family unit can be complex and hard to understand, so time at the alcohol rehab is needed. For the patient, meeting within the gay friendly alcohol rehab can provide them support as they come face to face with some of the shame they have carried for years.

Did 25 minutes on the stationary bike, 5 minutes on the mini trampoline and 20 minutes on the tread mill.

Wow! I didn?t know I could do that much after my Heart Attack. I never did that before?.

When I get scared I can talk to one of them, or the registered nurse and I feel connected to the group.

Isolation at a time when you are dealing with life altering experiences is the worst thing for us as humans.

I believe we are a social people and need to reach out to others. It is a skill really?.

Most of us are givers and don?t know how to be receivers.

Since my heart attack I have learned to accept help, I talk to my children more about meaningful topics and I ask their opinion.

I think the lesson in all this has been to open up our relationship, be more tolerant and certainly more grateful and reflective.

You get to think about things when your at rehab doing the work. So that?s another benefit of going.

A Rehabilitation "Gotcha": When 100 Days Isn't 100 Days

When her mother had a stroke Sandy didn't know the first thing about how Medicare defines benefits and "Rehabilitation Days." Because she didn't know how the system works she may have made some costly mistakes when her mother was discharged from the hospital to a skilled nursing facility for rehabilitation.

What Sandy discovered about how Medicare counts hospital days and benefit periods was important, because it had a direct impact on how much treatment her mother would be entitled to. The potential impact of Medicare rules needed to be taken into account when Sandy made plans for her mother's care.

Edith, Sandy's mother, spent a several days in the hospital after her stroke. The therapists then recommended that Edith move to a skilled nursing facility for more therapy. Edith was improving, but she needed more rehabilitation.

Edith had a good secondary insurance policy, so Sandy wasn't worried about her mother's rehabilitation being covered. She knew Medicare would cover up to a maximum of 100 days of inpatient therapy. When the rehab center said they were ready to send Edith home, Sandy advocated tirelessly for her mother to be allowed to continue therapy. Sandy ultimately convinced the center to keep Edith several more weeks.

A little more than nine weeks after what would have been her original rehabilitation discharge date Edith had a set-back when she fell at the rehab center. She suffered a hairline fracture of her hip and went back to the hospital for a few days.

Even though this second time in the hospital was for a brand new medical problem, Edith hadn't been out of the hospital and rehab for 60 consecutive days, so her re-hospitalization was considered to be a continuation of her first stay. After this second hospital stay Edith could probably have gone directly home, but Sandy again advocated strongly for what she thought her mother needed, and Edith returned to the rehab center for more physical therapy.

She is still unstable when she walks and needs to be watched closely so she doesn't fall again. She has now used up almost all of her 100 maximum allowable rehabilitation days under Medicare. She is still in her first Hospital Benefit Period because she has not been out of the hospital or rehabilitation facility for at least 60 consecutive days.

Now some hard decisions have to be made. Edith could stay on in the skilled nursing center for another week or two to take advantage of all the therapy she can get. That might make all the difference in preventing another fall. But that would also use up all of Edith's remaining Medicare-covered rehabilitation days. Or, she could go home and begin working on the 60 days she has to remain out of a medical facility. She could try to re-set her eligibility clock at home and keep those few remaining days available, just in case.

Whatever Sandy decides for her mother, they will be taking a gamble.

Not understanding how the Medicare system really works can have serious consequences for both the patient and the family.

How Medicare Benefit Periods Work

When it comes to the hospital (Medicare Part A), Medicare works by "Benefit Periods" rather than on an annual basis like most health insurance. After a hospitalization the patient must stay out of the hospital or other Medicare facility (like a rehabilitation or skilled nursing facility) for at least 60 consecutive days in order to re-set the clock and earn eligibility for a new Medicare Benefit Period.

If the patient goes back to the hospital within 60 days of being discharged from a Medicare facility (hospital or skilled nursing/ rehabilitation), it is considered to be part of the first hospital period, even if it is for a new medical problem. This has serious consequences for allowable rehabilitation days, as we saw with Edith. The second time she came out of the hospital, rather than starting fresh with another 100 potential days, as Sandy thought, she picked up her Medicare rehabilitation days where she left off.

When she has used up all of her allotted days, any additional skilled care Edith needs outside of the hospital will have to be paid out of her own pocket until she has been out of the Medicare inpatient system for at least 60 days.

So when Sandy thought she was helping her mother by pushing to extend her stay in the rehab facility, she was using precious rehabilitation days that she could possibly have saved. Edith could have been out of the "system" for more than 60 days when she fell, and she could have had a new Benefit Period. I really fault the rehab staff for not making this clear to Sandy, but here we are.

I recently received a call from a man who was interested in locating a gay drug rehab. He and his partner were planning a wedding and thought it would be a good idea to get clean and sober before going any further with their lives. Due to the fact that there is so much anger and shame within the family unit a drug rehab or gay drug rehab needs needs to have a strong family component. The family therapist needs to be skilled to keep addiction treatment from dissolving into the same kind of communication the couple has at home.

Recently a topic of conversation centered on whether there is a difference between a drug rehab and an addiction treatment center. As surprised as people were at the question, it made all of us realize those individuals without knowledge of the addiction treatment business or who have not been in addiction treatment themselves, might be easily confused.

There really is no difference between the two as far as the terminology. When a person mentions drug rehab, they are really referring to a drug rehabilitation program, which is the same as an addiction treatment program, alcohol treatment program or chemical dependency program. Addiction is sad, really. The life of an addict is often times harsh in ways that are not always visible. Of course, a lot of times the effects of addiction are visible.

Addiction is something that can hurt a great deal. Whether you are the person addicted or the loved one of someone with an addiction it can be an item of major concern that can manifest in multiple problems. You should seek to overcome the addiction as quickly as possible.

If you are the loved one, I hope you have the strength to stand by the person recovering or more importantly the one that hasn't begun to recover yet. The first step is for the person to recognize the problem. This can be tough, very tough on you. Maybe the worst thing you could do is to abandon that person. You could be the one that keeps him or her from completely falling off the edge.

I'm not an expert on addiction, so you can take all of this with a grain of salt. I'm only approaching things from a level of common sense that is uniquely my own. So take all of this with some degree of skepticism and consult a professional for proper care.

My personal beliefs hinge around compassion. If the one I loved was seriously addicted, I'm not sure I'd be able to get over my guilt if I abandoned that person. If they took a turn for the worse I would always wonder, "what if?".

No doubt about it, addiction brings great pain upon the afflicted and those closest to him or her. Consider that, think about it and mull over what that may mean to you personally and the people you love.

If you are the person with the addiction, or you are not sure if you really are addicted, think about it carefully. Is your substance use causing pain to the people you are closest too? Even if you are causing them only a little pain, you need to recognize that you may be addicted and need help.

Drug treatment and alcohol treatment rehab centers exist everywhere. Consider staying at one for a while. You've got to take a sober and serious look at your situation. Do you value your life and your loved ones more than your addiction? I hope the answer is that you cherish your family most. This is not your typical surface level question where you can get away with saying what you know people want to hear. You have to seriously take a look at yourself and come up with an honest answer.

If you treasure your relationships to others more, then it is time to give up the addiction. Life abounds with people who chose their addictions over their relationships. Their stories are all very sad. One thing is for sure: If you don't take action, the addiction will rule your destiny until you reclaim it.

Those around you may not be willing to wait around to see if you will change or not. Talk to them honestly, admit your addiction and then get help. There is no shame in that, instead it shows that you have some integrity and a strong enough will that may allow you to come back to the land of the living.

Since the beginning of time, man has been looking for the secret cure, the magic bullet, the fountain of youth that will solve all of our health problems. The truth of the matter is; There is no cure for some of life?s passages such as aging, diabetic problems or even Alcoholism or Drug Addiction. They can be controlled, even the quality of life may be improved, but they can not be completely eliminated. The clich?s appear hot and heavy with sayings like ?One Day at a Time? and a ?Journey of a Thousand miles begins with just one step? and so it is with Alcohol Rehab and finding your way home.

There are so many claims, so many schemes and so many quick fixes that it becomes almost impossible to find an Alcohol Rehab program that really works. Even the United States Government got into the act and spent hundreds of Millions of Dollars to find the magic cure for Alcohol and Drug abuse. After Seven years, a few hundred million dollars later, the PROJECT MATCH STUDY was deemed a dismal failure. Everybody and I do mean everybody, lined up to discredit, find fault in it and basically deny everything from the legitimacy of the parents of the authors to downright fraud. The truth of the matter was simply they did not like the results. And the results were dismal.

A funny thing happen however while everyone and their brother were circling the wagons and preparing to fight off the Calvary; Someone discovered by combining a combination of the protocols, that the success rate got better. At first, it was simply incorporating a 28 day in-residence alcohol rehab center with say follow up with AA meetings. The results jumped rather dramatically. In some cases, it climbed to as high as 73% while other studies showed an increase to higher than 75%. They were on to something. Add a couple of more ingredients such as educational protocol and behavioral modification and the numbers went into the Eighties. A non-profit by the name of Manor House Rehab went so far as to combine all known treatment protocols and used an extensive intake sheet to design an individual treatment and they hit over a 90% success rate for a temporary time frame until the results creeped back to ?normal? at about the 83-85% success rate. Not so bad.

The search for a perfect cure for Alcohol and Drug abuse had finally come to pass and success was at hand but only with a combination or a ?cocktail? of various programs combined to achieve success. It incorporated individual one on one counseling; It utilized group meetings, AA meetings and even a mentor program of one drunk or Addict helping another drunk find his way out of the darkness. They stole a few ideas from AA and included a higher power into their program, they copied Zen and Meditation from the East, basically went out and tried anything and everything that seemed to have a chance at success. The more they tried, the more successful their results were.

It sort of rambles on for about ten pages, but it makes the point quite well that a combination of many factors contribute to the success or failure of an Alcohol Rehab Program. Locating a facility on the beach may not be such a good idea because of triggers and cravings. To drink or do drugs is present simply because a beach party seems to be conducive to getting drunk or high or a contributing factor. The Manor House Rehab idea of an Ideal Alcohol Rehab center basically was on a desert Island, a thousand miles from nowhere without any booze or drugs anywhere to be found. Not a bad philosophy since Rehab is not a vacation, not Club Med, but Rehab and changing some of the habits that lead to getting stoned or drunk is the results intended, not a party. For the sake of full disclosure, I guess I should admit that I contributed heavily to the Manor House article, that I am a member of ?those other guys? with professional membership in NAADAC (National Association of Alcohol and Drug Addiction Counselors), graduate degree?s and about 25 years experience in grief counseling, addiction counseling and spiritual guidance. I decided not to present a professional paper for publication but to state my personal opinion as opposed to my professional opinion that basically, most Alcohol and Drug Rehab Centers got it all wrong. In a moment of weakness, I had to admit that the Medical Model of Alcohol Rehab pretty much screwed things up for awhile. It can be quite difficult to master this so be sure to practice. If you have difficulty visualising the 'upwards' and 'resting' states, it helps to practice on the toilet. When you begin urinating try and stop the flow of urine midflow then release. This is the same as what you want to be doing during your exercises.

Once you have mastered your pelvic floor exercise of tightening and releasing, then it is time to introduce a dilator. A dilator is essential a small tube,, it comes in various sizes. The greater your pain, the smaller the size of the dilator you should begin with. If you do not have a dilator you can use a clean finger.

Step 1: Apply a small amount of lubricant to the tip of your dilator.

Step 2. Following the same steps as above, lie down on your back with your knees bent. Relax your pelvic floor muscles and insert the dilator as deep as is comfortable.

Step 3. Tighten your pelvic floor muscles around the dilator. Remember to stay relaxed and take deep breathes.

Step 4. Rotate the dilator. Imagine a clock face, hold the dilator in the directions of 3 O'clock, 6 O'clock, 9 O'clock and 12 O'clock.

Step 4. Hold each for 5 seconds then release and rest for 10 seconds.

Step 5. Repeat 10 times.

When first starting it can help to use your finger instead of the dilator. This way you can actually feel your pelvic muscles tightening around your finger then releasing, so you know that you are doing the exercise correctly.

As your pain decreases, go up a size until you are ready to introduce your partner into the exercises and resume sexual contact.
